Bladder Stones

Bladder stones are solid masses composed of minerals that form within the urinary bladder. They typically arise when urine remains stagnant in the bladder for extended periods. While smaller stones might go unnoticed and pass naturally, larger ones can lead to abdominal pain, difficulty urinating, and even blood in the urine. If unable to pass the stone naturally, an urologist can employ methods like breaking it down with a laser or surgically removing it.

What Are Bladder Stones?

Bladder stones, also known as bladder calculi, are solid mineral formations that develop in the urinary bladder, where urine is stored after passing through the urinary system.

Symptoms and Causes

Bladder stones may not cause noticeable symptoms when they are small, often being expelled during urination without any issues. However, larger stones can irritate the bladder, resulting in severe pain, bleeding, and difficulties in urination. Common symptoms of larger bladder stones include changes in urine color, frequent urges to urinate, painful urination, and urinary tract infections (UTIs).

The primary cause of bladder stone formation is the concentration of waste products (such as salt, potassium, and protein waste) in the bladder due to incomplete emptying during urination.

Certain factors increase the risk of developing bladder stones, such as bladder augmentation procedures, bladder diverticula (pockets in the bladder that trap urine), dropped bladder (cystocele), dehydration, enlarged prostate, kidney stones, neurogenic bladder (nerve damage affecting bladder function), and the prolonged presence of medical devices like catheters in the bladder.

Diagnosis and Tests

To diagnose bladder stones, we review medical history, assess symptoms, and conduct physical examinations. Diagnostic tests may include urinalysis to check for signs of stones or UTIs, imaging tests like CT scans, X-rays, and ultrasounds to visualize the bladder and stones, and cystoscopy, which involves using a thin tube with a camera to inspect the bladder’s interior.

Common treatment options include:

  1. Cystolitholapaxy: This minimally invasive procedure employs a cystoscope to locate the bladder stone and then break it into smaller pieces using a laser or high-frequency sound waves. The smaller pieces are then washed out of the bladder.
  2. Surgery: For larger stones, open surgery may be necessary to remove them. An incision is made in the abdomen to extract the stone, and in cases related to benign prostate hyperplasia, prostate tissue that obstructs the urethra might also be removed.

 Prevention

While it may not always be possible to prevent bladder stones, certain measures can help reduce the risk. Drinking plenty of water can dilute minerals in urine, making them less likely to form stones. For individuals over 50 with an enlarged prostate, seeking medical advice on bladder emptying techniques and medications can be beneficial. Additionally, modifying one’s diet and taking specific medications can aid in lowering the risk of certain types of stones.
